Low-Grade B-cell Malignancy with M-Protein and Autoimmune Cytopenia

リンパ球系腫瘍ではしばしば自己免疫性血球減少（AIC）を合併する．しかし，どのような特徴をもつ症例で合併しやすいかは明らかでない．当院で診断した低悪性度B細胞腫瘍（LGBCM）155例を対象とし，immunoglobulin（Ig）の増加と，AICの合併頻度を後方視的に検討した．6例がAICを合併し，そのうち4例でIgが増加していた．4例中3例がmonoclonal（Mono）IgGを伴い，その3例中1例はpolyclonal（Poly）なIgM増加を伴った．その他の1例ではPoly-IgM増加のみを認めた．Mono-IgGを伴う症例では，AICの合併が多かった（p＝0.018）．一方，Poly-Ig増加や，Mono-IgMおよびMono-IgAを伴う症例では，AIC合併の増加はなかった．Mono-IgGを伴うLGBCMでは，AIC合併に留意して診療を行う必要があると考えられた．
